PREVALENCE HELP
Prevalence is defined as a count of beneficiaries with the selected chronic condition divided by the count of all beneficiaries, in percentage format rounded to the nearest hundredth place.
You can select the specific chronic condition information displayed in the map, table, and bar chart from the drop down menu above the table. By selecting a state on the map, you can change the selection of county information presented in the bar graph at the bottom of the page.
Information is suppressed that is based upon fewer than eleven (11) beneficiaries in the population.
Chronic Condition Prevalence: State to National Ratio Map (Left Map)This map shows the prevalence of beneficiaries with a selected chronic condition presented as a ratio of the state to the national average. You can hover-over each state and the District of Columbia to show specific values. By selecting a state you determine the counties shown in the bar graph below.Chronic Condition Prevalence: Table (Right Table)This table shows the prevalence of beneficiaries with a selected chronic condition. The selector to the right of the table allows you to change the table view from state-level information to county ranks. The 10 highest and 10 lowest counties in terms of prevalence are displayed. Chronic Conditions Prevalence, Counties Bar Graph (Bottom Graph)This bar graph shows the prevalence of beneficiaries with a selected chronic condition for every county in the state selected from the map. You can hover-over each bar to show specific values for a county. You can select a county in the bar graph to display in the table to the right of the bar graph. Data can be viewed in a table format by changing the selector to the right of the title from “Graph” to “Grid.”
SPENDING HELP
Prevalence, Medicare spending and utilization information is presented by the number of chronic conditions, grouped into 4 categories. You can select the number of chronic conditions displayed in the map, table, and bubble chart from the drop down menu above the table. By selecting a state on the map, you can change the selection of county information presented in the bubble graph at the bottom of the page.
Prevalence is defined as a count of beneficiaries with the selected number of chronic conditions divided by the count of all beneficiaries, in percentage format rounded to the nearest hundredth place. Per capita Medicare spending is defined as the total of all standardized payments (Parts A and B) for beneficiaries with the selected number of chronic conditions divided by the count of beneficiaries with the selected number of chronic conditions, in currency format rounded to the nearest whole dollar. Within each chronic condition count grouping, the number of ED visits per 1,000 Medicare beneficiaries, is defined as the total number of emergency room visits divided by the count of beneficiaries times 1000, in numeric format rounded to the nearest whole number. Within each chronic condition count grouping, the 30 day all-cause hospital readmission rate is defined as the count of readmissions to the same hospital occurring within 30 days after discharge from the initial visit divided by the count of admissions, in percentage format rounded to the nearest hundredth place.
